As part of providing support, we need your consent to collect and record personal information from you that is relevant to your situation. Your personal information will be recorded in a secure database called electronic medical records called Best Practice. If you see the GP at headspace, we will also need to share your Medicare details to bulk bill Medicare for the GP services.

Is to provide you with the best possible support, care, and treatment, and to continue to improve our services; we will need to collect information about the young people who access our services, the services they receive and the outcomes they achieve.
headspace Mount Isa will collect personal information that helps us provide services to you which will include your personal details, address, and contact number; your gender and sexual identity; sexuality; emergency contact and ethnic identity.
When you talk with us, it is important that you feel safe and comfortable. You might prefer to seek support from someone of the same gender or someone who understands your cultural background. We will do our best to make sure this happens.
sharing and use of information:
Prior to your session at headspace Mount Isa, who will receive a link to a survey called headspace Application Platform Interface – HAPI, to complete the survey before you arrive at the centre. You can also complete it in the centre using a headspace iPad. The survey helps us track how you are doing and supports headspace Mount Isa with ongoing funding.
1. We use it for providing healthcare services:
The information you provide will be used by staff at the headspace centre you visit to help them provide you with an appropriate service. This means that headspace centre staff now and in the future are able to look at the information you and they provide. There are also some rare occasions when information you have provided to staff has to be disclosed for legal reasons e.g., a subpoena or court order. This will be explained to you by your intake worker, care coordinator or clinician.
2. We use and share it for evaluating, reporting on, and planning our services:
The information will be used by headspace Mount Isa and headspace National to evaluate and report on how well the centre is providing health services to young people and to help staff plan the services they provide to you.
These activities will help headspace understand the characteristics and needs of young people using headspace services to make headspace services better.
We sometimes engage partners to conduct evaluation, research and reporting activities using information we provide. The information disclosed by headspace to these partners will always be de-identified, which means that the information provided will not include your name and will not identify you as an individual.
3. We provide information to other agencies to allow them to monitor and evaluate headspace services:
headspace centres are funded by Primary Health Networks (PHNs) across Australia. PHNs are funded by the Australian Department of Health. The Department of Health regularly reviews and evaluates the services provided at headspace centres with the aim of improving the services that are provided to you and other young people. headspace Mount Isa provides de-identified information to the Western Queensland Primary Health Network (WQPHN) for the monitoring, evaluation, and service planning purposes of the PHNs.
As part of the accreditation and quality assurances purposes for our lead agency – Mount Isa Aboriginal Community Controlled Health Service Limited t/a Gidgee Healing, we provide de-identified information for monitoring and evaluation and for service planning and funding purposes for the Northwest Queensland region.

This will be explained to me that in depth when I have my initial contact:
- There are some instances where headspace Mount Isa worker may have to break confidentiality to keep me safe.
- That headspace staff meet regularly to discuss information relating to me to give me the best care outcome!
- headspace Mount Isa is a voluntary service, and I can choose to leave and not come back at any time.
- If I have any worries about the service I receive, I can talk to my worker, any headspace Mount Isa worker, or the manager at headspace Mount Isa.
- Each time I have an appointment with my headspace worker, I will complete a hapi survey. The information from this survey is used by my headspace worker to assist in providing me with the best support possible.
- I understand that the information I provide to headspace will remain confidential except in the above-mentioned situations and according to the headspace Privacy Policy.
- I understand that the information shared with headspace National, PHNs, lead agency and other partners to assist with evaluation, research and funding is made anonymous.
my responsibilities are:
- Treat staff and other young people without discrimination.
- Attend your headspace appointment without the influence of alcohol or other drugs.
- Call if you are running late or cannot attend your appointment.
- Let us know if you no longer wish to access headspace services.
confidentiality and privacy:
headspace Mount Isa is committed to the protection of your privacy and managing your personal information in accordance with the law, including securely storing the information we collect from you. The headspace Mount Isa staff sign a confidentiality agreement where they cannot talk about you or your personal information outside of work.
Duty of Care
We will not disclose your personal information to any third parties without your consent, unless permitted or required to do so by law, when we exercise our Duty of Care.
These are some exceptions when we are obliged to try to protect you and/or others if the information you submit tells us that:
- you are at risk of harming yourself or someone else.
- you are or have been abused by someone and you are under the age of 18.
- you are being subpoenaed by a Court.
We will always try to tell you first if we need to break your confidentiality.
If you are under 16, we will also talk to you about what will be discussed with your parent/guardian/carer and the extent to which they will be involved in your care at headspace Mount Isa.
