  • The intention of this agreement is to assure you of our deepest appreciation of your services and to indicate our commitment to do the very best we can to make your volunteer experience a productive and rewarding one. Only members in good standing with the NAPO Chicago Chapter may participate as volunteers.

  • and we commit to the following:

    1. The leader of the project will provide adequate information, training, and assistance for the volunteer to be able to meet the responsibilities of their volunteer position.
    2. To ensure satisfactory supervisory support of the volunteer and to provide feedback on performance by the leader of this project.
    3. To respect the skills, dignity and individual needs of the volunteer, and to do our best to adjust to these individual requirements.
    4. To be receptive to any comments from the volunteer regarding ways in which we might mutually better accomplish the respective tasks.

    I, the VOLUNTEER, agree to serve as volunteer and commit to the following:

    1. To perform my volunteer duties to the best if my ability.
    2. To ask for help or support when needed.
    3. To provide reports or feedback, communicating relevant and important information to the appropriate board member or leader, as requested.
    4. To assume all risks in working under this agreement, and to assume sole responsibility and answer for all damages or injuries; I waive, release and discharge and hold NAPO Chicago Chapter and its directors, officers, members, and employees harmless from all liability, including, but not limited to, personal injury and property damage arising from or related in any way to my work as a volunteer; including my traveling to and from the event, whether arising from the negligence or fault of NAPO Chicago Chapter or any of its directors, officers, members, employees, or other volunteers, or otherwise.
    5. To adhere to the NAPO Chicago Chapter policies and procedures and 1) agree to abide by the NAPO Code of Ethics, and 2) respect confidentiality of the client, public and peer volunteers.
    6. To wear the appropriate attire as determined by either the NAPO Chicago Chapter or project leader.
    7. All projects are considered to be “clients” of NAPO Chicago Chapter and as such, no volunteer will provide personal or company materials while volunteering for this event. Any requests for information shall be answered with directions to the NAPO Chicago Chapter website.
    8. Volunteer position does not entitle volunteer to compensation as in the form of payment.

    Failure to adhere to the terms of this agreement will result in forfeiture of volunteer status. Signature below signifies acceptance of this agreement.

  • NAPO Chicago Photo/Video Release (Adult)

  • I acknowledge that the NAPO Chicago, a chapter of the National Association of Productivity and Organizing Professionals, is requesting my permission, and through this release and in consideration of having the opportunity to have my photographic and/or video image, as well as a photographic and/or video image of my property, which I testify that I own, if applicable, utilized by NAPO Chicago. I am granting NAPO Chicago such permission, to use my photography and/or video and photographs and/or video of my property, if applicable, in projects related to promoting the Association and the productivity and organizing industry. These projects may include, but are not limited to, Get Organized Month materials, educational material about the profession NAPO Chicago’s website and their social media.


    I hereby give to NAPO Chicago the right and permission to use my photographic and/or video image(s) and photographic and/or video image(s) of my property, if applicable. I waive any right to inspect or approve the finished copy, images or printed matter that may be created in conjunction with this material. I also agree that NAPO Chicago shall be without liability for me for any distortion or illusionary effect resulting from the publication of my photographic image and that nothing in this Release requires NAPO Chicago to make any use of the rights it is acquiring.


    I represent that this agreement does not in any way conflict with any other existing commitment on my part.


    I have read the forgoing release agreement before affixing my signature below and certify that I fully understand the contents of this release.

  • Waiver and Release of Liability by Event Participant

    READ CAREFULLY -- BY SIGNING THIS DOCUMENT YOU MAY GIVE UP IMPORTANT LEGAL RIGHTS
  • I may be exposed to persons who, knowingly or unknowingly, are infected with the novel coronavirus and/or have contracted COVID-19. The waiver, release and other representations set forth herein are given in consideration for the Sponsor and the Venue permitting me to participate in the Event.

    1. Acceptance of Risk; Release; Indemnification. I am fully aware that there are a number of risks associated with me entering onto the Venue property and participating in the Event during the COVID-19 pandemic, including without limitation: (a) I could contract COVID-19 or other diseases which could result in a serious medical condition requiring medical treatment in a hospital or which could possibly lead to death; and (b) I will be subject to normal risks associated
    with participating in the Event, such as physical injuries or even death or loss or damage to personal property, including without limitation, from slips or falls, food poisoning or allergic reaction to food served during the Event, physical or verbal altercations with Venue staff or other Event participants, or injury due to the nature of the Event. On behalf of myself and my heirs, successors and assigns, I knowingly and freely assume all such risks, both known and unknown, relating to my participation in the Event and being present at the Venue, and I hereby forever release, waive, relinquish and discharge the Sponsor, the Venue and any national organization with which Sponsor may be affiliated, along with their officers, directors, managers, officials, trustees, agents, employees or other representatives, and their successors and assigns (collectively, the “Sponsor and Venue Representatives”), from any and all claims, demands, liabilities, rights, damages, expenses, and causes of action of whatever kind or nature, and other losses of any kind, whether known or unknown, foreseen or unforeseen, (collectively, “Damages”) as a result of me being present on the Venue property and participating at the Event as described above, including but not limited to those related to the above described personal injuries, death, disease or property losses, or any other loss, and including but not limited to claims based on the alleged negligence of any Sponsor or Venue Representative or any other person. I further promise not to sue the Sponsor or the Venue or any Sponsor or Venue Representative, and agree to indemnify and hold them harmless from any and all Damages resulting from my being present at the Venue and participating in the Event.

    2. Affirmative Representations. I represent and warrant that I will not enter the Venue or participate in the Event if (a) I am aware, as of the date of the Event, that I have contracted and not completely recovered from COVID-19; (b) I am aware, as of the date of the Event, that I have been exposed within the prior two weeks to someone who had contracted or was subsequently determined to have contracted, COVID-19; (c) I have been tested for COVID-19 but not yet received the results of such test; or (d) I have symptoms consistent with having contracted COVID-19. I also represent and warrant that, during the course of the Event, I will comply with all applicable regulations and guidelines regarding COVID-19, including those imposed by the Venue and/or the Sponsor.
