Each Captain participating in a match under the jurisdiction of Auckland Cricket Association is required to complete this Report form at the end of each match. Please ensure this is submitted to Auckland Cricket by 5.00pm of the following Monday

As a guide, a mark of 3 is average and should be the starting point. A mark above 3 ranges to 5 for excellent and a mark below 3 indicates less than satisfactory conduct. 1 is very poor.
