Practical Evidence must be completed for the issue of a Certified Handlers Certificate. This evidence is to be a true record of your storage and spray systems. 

Template documents can be found on our website  - please feel free to print them off and use them. 

You can either attach all your documents and photos as 1 x document upload  or you can individually upload photographs and copies of your records as listed below. 
If possible please upload clear but low resolution photos. 

Please note: we are currently going through an internal audit and additional information/evidence maybe required at the time of checking.

This application form is a compulsory requirement for every application.
All sections of the form must be filled in.
