  • Medical and Health Details

  • YMCA SA works hard to provide safe, supported and enjoyable camps and activities. Please help us to meet this goal by providing us with as much information about medical, health, behavioural and dietary issues or concerns as you can.

  • DIETARY REQUIREMENTS

  • ALLERGIES

  • MEDICAL AND HEALTH INFORMATION

  • MEDICATION

  • A participant may not have any medications in their possession at camp. All medication must be given to, and held by a YMCA staff member, who will make the medications available to participants as per the written instructions on the container as prescribed by a doctor or physician.

This also includes over the counter medication like paracetamol (eg. Panadol) and cough lollies. All medications should be in a blister or webster-pak.

Please provide details with the medicine at the check in point for camp. This should include any changes since you completed this form. This should also include medication the participant has recently completed in case any side effects occur.

If there is parent/guardian permission, participants may take responsibility for their Inhalers and EpiPens in which case they should be kept on their person at all times.

No spot will be allocated on camp until a doctors certificate with the prescribed medications and dosage times are attached to an email and sent to sayouth@ymca.org.au

In previous camps we have had guardians and parents arriving to bus pickup points with new medications or dosage levels and no doctors certificate. We cannot accept this to occur again.

This is a risk assesment and safeguarding measure that has been put into place by the YMCA SA.

  • I give permission for the participant to be provided the following over the counter medications by YMCA staff if deemed appropriate.

  • SUPPORT AND PARTICIPATION AT CAMP

  • YMCA is committed to creating positive experiences for everyone on camp. We strongly believe camps should be more than a way to occupy the time, they should support people and build people up.

To assist us, please provide any information (such as behavioural support needs, health concerns, disability or diagnosis) that we need to be aware of in order to create this experience. Please provide more information in the boxes below.

Please also provide any relevant information about any conditions or issues that may affect your participation in this camp. This may also include support being needed for:

    • Showering
    • Toileting
    • Meal times
    • Dressing
    • Bed time
    • Encouraging appropriate social skills
    • If a camper may disclose sensitive information
    • Emotional support and encouragement
  • We will be in touch in regards to what aspects of camp and the camp fee we can use your NDIS funding for.

  • We take a strengths-based approach to our campers. This means we are all about building on their existing strengths and helping them to develop more! The following Strengths and Difficulties Questionnaire (SDQ) is an easy way for us to get a better understanding of our campers before we offically meet. The SDQ is a simple 25 item questionnaire that gives us an idea of areas that our campers are thriving in, and areas where they might need a little more help. It helps us support our young campers to the best of our ability if you are able to answer the questions as accurately as you can, using the last 6 weeks as a guide.
