First game kicks off at 8:30 AM, Pack up around 4:30 PM

Women’s grade – Must be over the age of 17 by 19th October 2019.

Secondary School Girls under 15 – Must be younger than 15 on the 1st January 2019.

$100 per team - invoices to be generated after registration is complete.

*All players are required to be ready 1min before their game(s) (boots & mouth guards are essential)

Liability Waver

I agree to all player’s participation in the tournament and accept that while all reasonable care will be taken by the HB Regional Sports Park, they cannot be held responsible should any injury occur. By registering your team you waiver and release all HB Regional Sports Park staff from liability for possible injuries and associated costs that may occur to your players whilst in attendance participating in the tournament.

Drugs & Substance Abuse

Under no circumstances is there consumption or taking of any drugs or any substance abuse permitted (regardless of type, amount and method taken), at any stage while in attendance and participating in the tournament. Should HB Regional Sports Park staff witness or be exposed to such a situation, instant dismissal from the competition shall result, with a proper and appropriate investigation to take place at a later point in time.

Social Media

By signing this form, I understand and give permission for our player’s images to be used on social media platforms by the HB Regional Sports Park and other professional sports photographers during the duration of the tournament.

Consent

By signing this entry form, I understand that failure to abide by the competition rules as above and not limited to, could result in the team being removed from the competition.

As the person in charge of the team, I have checked the medical status of each player and acknowledge they are eligible to play. Any player under the age of 18 has been given consent from their parent, guardian or caregiver.

We agree to support the staff of the HB Regional Sports Park, and we agree with, will adhere and consent to all of the above, namely the: Liability Waiver; Drugs, Smoking & Substance Abuse and Social Media.
