• Fraternity and Sorority Membership Update Form

    This form should be completed by the president of the fraternity or sorority chapter. Please use this form to submit any changes to the chapter membership of the fraternity or sorority for which you represent. This form should be submitted within 48 hours of the change(s).
  • Note: This form should not be used for members who have graduated and are are in an alumni chapter, if applicable.  Those members will automatically be removed at the start of the next term!

  • New or Initiated (Active) Members

    Please enter the type of member addition (new member or active member), name, Knox I.D. number, and the date they became a new or active member in the table below for each student to be added to the chapter roster.

    Please enter the name, Knox I.D. number, and the date the member(s) were granted special status from the chapter, or the date in which they returned to active member status.

  • Membership Deactivation (Remove Members)

    Please enter the name, Knox I.D. number, date of membership termination, and the reason for the termination in the table below for each student to be removed from the chapter roster.
