Animal Surrender Form
This form is used by Paws & Claws Adoptions for the surrender of animals.

It is essential that all owners read, understand and agree to the terms and conditions set out by the rescue.

If the owner has any questions regarding the surrender terms set on this form, the questions MUST be discuss and resolve with the surrender officer before signing.

All surrenders are conditional on Paws & Claws Adoptions finding a suitable placement for the animal(s).

Paws & Claws Adoptions charges a one off $50 fee per group of animals.
This surrender fee helps support the costs of rescuing and rehoming the animals.

Surrender Process
- Owner contacts Paws & Claws with their name, mobile, address, email, details of the animal(s) and reason they wish to surrender
- Surrender Officer assesses if the rescue has capacity to take on the animal(s)
- If the rescue has capacity, the Surrender Officer will attempt to find a placement with a carer if required
- Surrender Officer prefills most of the information on this form and then send to the owner to complete
- Once completed, a Paws & Claws volunteer will be arranged to collect the animal(s)

Animal Surrender Form - Conditions of Surrender
THIS AGREEMENT is made between Paws & Claws Adoptions Inc. (PAC) and the mention owner(s).

I/ we accept, acknowledge and agree that
1. I/ we are the legal owner(s) of the above animal;
2. by signing this document, I/ we relinquish any claim, right, title or interest in the animal to Paws & Claws
  Adoptions, who can exercise full discretion as to the future care and management of the animal;
3. I/ we are signing this document of my/ our own free will;
4. Paws & Claws Adoptions can in its absolute discretion exercise any right(s) in relation to this animal, including
  the right to permanently re-home the same; and
5. I/ we will pay a surrender fee of $50.00 per group of animals, which fee supports Paws and Claws Adoptions to
  continue with their work in rescuing and re-homing animals
