What do you want to achieve?
Knowing your support needs and goals are important first steps in preparing for your NDIS plan. Understanding this in detail will help us to outline the things that are essential to your daily life and ensure that your plan helps you to achieve your future goals.
So let’s get started! The most important way for you to prepare for your meeting with the NDIA is to provide information on:
- The important people in your life who provide unpaid support
- Your service providers who provide paid support
- The aids and equipment you currently make use of
- Your living arrangements
- Your regular activities, such as social interests, education and work
- What is working well in your life
- Where you could use a little extra support
There are 4 sections to complete in this workbook. We will begin with your current living situation and move through to what you may require in the future.
