Congress MusicFactory Registration

As part of the process of managing Congress MusicFactory Contributors, Congress WBN needs to collect personal information from you. Personal information is information that we hold about you such as your name and email address. We want you to have control over how your personal information is used and under the General Data Protection Regulation we need to get your agreement to continue to use your personal information. Therefore you should use this form to tell us how we can continue to use your personal information.

If you have parental responsibility for children under the age of thirteen years old, you also need to complete this form for each child registering as a contributor. If you are age thirteen years old or over you should complete this form personally.
