CUSTOMER AUTHORISATION FORM
1 FIBRE BROADBAND GET CONNECTED
We are seeking your authorisations described below to enable your household to be connected to and have the advantages of fibre broadband. In seeking such authorisations, we are providing you with information about connecting to Enable Networks Ltd’s (“Enable”) fibre broadband network and ordering fibre broadband services from a retail service provider (“RSP”) of your choosing. In providing you with this information, we are not acting as agents of Enable nor are we promoting any RSP over another RSP. We are an independent contractor engaged by Enable to promote the benefits of Christchurch’s own fibre broadband. Enable does not sell its fibre broadband directly to you and if you wish to acquire Enable’s services, you must purchase your fibre broadband directly through a RSP. Our role is to promote Enable’s fibre broadband services by describing the benefits of fibre broadband – we do not supply any of the services ourselves. You are not required to make any payment to us for our advice.
2 YOUR AUTHORISATION
By signing this form, you confirm that:
1. You wish to have your household connected to the Enable fibre broadband network, and you therefore authorise us to arrange for Enable to install a fibre connection into your house at no cost to you. Enable will contact you to arrange a date to visit you to assess how best to install a fibre connection into your residence.
2. You intend to look into retail options and request a broadband service from a RSP of your choice, and you authorise us to contact you from time to time to monitor progress and/or provide further advice on connecting with a RSP of your choice.

In order to facilitate the Authorisations, you agree that we and Enable may contact you by any reasonable method, including face to face, correspondence, email and telephone.
3 INFORMATION ABOUT YOU
Our service involves the collection by us of personal information about you, such as your email address, name, home or work address and telephone number. You may ask to see any information we hold about you and we will provide it as long as we have such information. You may also ask us to correct any incorrect information we hold about you. We may provide your personal information to Enable as authorised by you above.
Your Authorisation:
I authorise Savvy Marketing Limited to perform the activities described in the Authorisations. By executing this Customer Authorisation, I warrant that I am authorised to sign this Customer Authorisation as either property owner or authorised agent of the property owner.
