Cancer Council's Transport to Treatment program aims to assist those people in greatest need in the community affected by cancer who are needing to access cancer related appointments/treatment at Dubbo Base Hospital. Eligibility for services will be assessed using the checklist below and will be determined by the coordinator using their discretion. Clients should exhaust all other transport options, such as family/friends/community transport prior to requesting this service.
As resources are limited, not all clients who are eligible will be able to receive support. Coordinators are responsible for the prioritisation of client's need for service and appropriate allocation of resources. The coordinator will keep a record of all requests for this service.
Hours of operation are Monday - Friday, 7:30am - 6pm.
Please note requests for transport should be lodged with Cancer Council NSW Orange office by 5.00pm Wednesdays', for transport during or commencing the following week.
When you have entered all the trip and client details, please scroll to the bottom of the page and click on SUBMIT.
