Before submitting your application to withdraw, please take some time to review the Policy on Former Trainees Seeking to Reregister.

Please ensure that you print any copies of assessments prior to completing the online request form as once you are no longer an ACEM trainee these assessments are no longer accessible. To obtain these assessments once you have been withdrawn, an administrative fee would apply.

Depending on the training requirements and assessments completed, you can receive Recognition of Prior Learning (RPL) for various components of the Diploma, or Certificate, giving you the opportunity to still receive a formal qualification in Emergency Medicine.

We would appreciate it if you could provide feedback on your thoughts and experiences as an ACEM trainee, by answering the following two questions. This feedback will assist us in identifying the strengths and weaknesses of the training program and to understand the issues experienced by trainees. This information will be used for quality assurance purposes, to inform and improve the trainee program and experience.

1. I understand that the withdrawal is effective from the date of the submission of this form to the College and no further training will be accredited to my training record from this date.

2. I understand that I will no longer be enrolled in the FACEM Training Program and will not have access to the members only content on the ACEM website.

3. I understand that any documentation requested from my trainee file post withdrawal will require payment of an administration fee.

The College will notify you once your application to withdraw has been finalised.
