By enrolling, you acknowledge that you have read, understood, and agree to the Policies & Consents relating to uniforms, fees and payment plans, photo and video usage, end-of-year production, exams, adult classes, and Ready Set Dance.
As part of your Annual Concert Levy, you will receive a cast T-shirt. Please indicate your preferred size in the box below, and specify any additional shirts you would like to order for yourself or other family members at an additional cost of $35 each.

**IMPORTANT** I am aware that my child is NOT to wear their Concert shirts to regular classes in Terms 1, 2 & 3 as they are not part of the uniform. Please refer to the Uniform Policy for the correct attire for each genre.
