    IMPORTANT INFORMATION:

    The purpose of this Middle Leadership review template is to provide assistance to schools when conducting a performance review at the conclusion of the appointment period as provided for in Schedule 4.14 of the Catholic Employing Authorities single enterprise collective agreements.

    These templates are not intended to be used for managing underperformance processes.

    Further information relating to the responsibilities, attributes and duties for Curriculum Leaders in secondary schools, can be found at Table 5 of Schedule 4 of the Agreement.

    What if someone holds more than one (1) Middle Leader position?

    Separate appraisal templates (one for each Middle Leader area) will need to be completed for each position.  For example:  If a person holds a Curriculum Leader position and a Pastoral Leader position, the Curriculum Leader appraisal template and the Pastoral Leader appraisal template will both need to be completed.

    INSTRUCTIONS:  PLEASE READ CAREFULLY PRIOR TO COMPLETING THE TEMPLATE

    You are currently in the Curriculum Leader appraisal template. Please refer to the other link/s if the appraisee is either a Pastoral Leader or Program Leader, or holds more than one Middle Leader position.

    1 Enter appraisee’s first and last name
    2 Use the drop-down box to nominate the relevant Faculty
    3 School identifier – commence typing school name and a list of schools will appear
    4 The date will automatically default to the date the template is opened
    5 Select the appropriate Tier level from the drop-down box
    6 Each category (Curriculum, Leadership and Management) is represented by a coloured box and contains a list of typical duties relevant to that aspect of the appraisal.  Each box can be expanded or hidden by clicking on the arrow.
    7 Appraisal can be partially completed and resumed at a later time, by closing the template and re-opening it from the link.

    The completed template can be saved through Adobe as a PDF for electronic filing.

    TO SAVE AS AN ELECTRONIC FILE

    Press the green “Save as PDF” button on the template. Click “Change” in the “Destination” area under the Print/Cancel buttons. Locate and click on the “Adobe PDF” setting from the drop-down options, then click the “Print” button on your computer. This will trigger an option to save the form in the preferred location on your internal file management system. Click “Save”. The PDF document will appear on your screen shortly after and can be closed, having already been saved to your system.

    Once the document has been saved to your internal file management system, hit the “Completed” button to reset the form. When you click on the “Completed” button, you will be prompted by a request to verify whether you are “OK” to clear the form of all data (to save, see step 8) or “Cancel” to continue working in the template. 

    NOTE: Once you have clicked “OK” all data will be cleared from the template. This information cannot be retrieved once it has been cleared.
