We support individuals who wish to:

5. require assistance with health or medical needs.

The organisation must be supplying or providing a service or activity that is open to all.

Projects considered will be, but are not limited to:

a. educational;

b. environmental;

c. heritage preservation;

d. sporting;

e. recreational;

f. ethnic and cultural;

g. arts, dance and music; or

h. groups supporting the most vulnerable in the community.

Before you commence the application process ensure you have:

· information available to complete the application form;

· a deposit slip in the name of the applicant organisation;

· any supporting information handy.

Please allow at least 6 weeks turn-a- round from the date you submitted your application. We will review your application and get back to you as soon as possible. You will also be emailed a copy of the information submitted in this form upon submission.
