  • Congratulations on your pregnancy and welcome to Mater Mothers’ Hospital. To allow us to prepare for your first antenatal appointment please complete the following online Antenatal History form below. The information you provide us will be available to your midwife and doctor at your appointment and therefore may reduce the length of time required for this appointment. Please complete and submit your Antenatal History via this online form within 2 business days. Please note that this form does not replace your first appointment and you will still be required to attend your appointment in order to complete your maternity booking.

     To access the form you will need to enter:

    • Your Patient Reference Number (PRN) - included in the SMS we sent you
    • Your E-mail address

    The form will require you to answer questions about your medical, surgical and pregnancy history, so you will need to have the following information ready:

      • Previous pregnancy information
      • Family health history
      • Your height and weight
      • A list of your current medications

    You can choose to complete your Antenatal History on your mobile device, computer or tablet. Once you complete details about any previous pregnancies, an email will be automatically generated and sent to the address you provided. Your online Antenatal History will be saved as you go allowing you to return to complete it at your own convenience. To re-access your Antenatal History simply click the link in the email. If you do not find the email in your inbox, please check your spam/junk mail folder.

    If you experience difficulties with completing your Antenatal History, please phone the Antenatal Clinic on 07 3163 8330 as early as possible between 8am and 4pm, Monday to Friday.

    • What is my patient reference number (PRN)? 
    • What is a PRN number?

       

      A PRN number is a Patient Record Number that is assigned to you by Mater Health. Your PRN is unique to you and is used to help correctly identify you each time you have any interaction with Mater Health.  Your PRN cannot be re-used by any other person.

       

      How do I know what my PRN number is?

       

      Your PRN should be included in the text message you received from Mater Mothers informing you of this online Antenatal History.  Your PRN will also be included at the top of any letters that you may receive from Mater Mothers.

       If you have not received your PRN please contact the Antenatal Clinic as soon as possible on 3163 8330 between 8am and 4pm, Monday to Friday.
