    Please use this form if an insurer refused to pay fair and reasonable freight charges, either partially or fully. Please ensure the charges have been reasonably incurred by you as the repairer as part of the repair process.

    • I have read and understood the Motor Vehicle Insurance Repair Industry Conduct of Conduct commencing 1 May 2017, and note my obligations and those of insurers detailed under the Code. I say that the insurer has breached its obligations under the following clauses of the Code: Clause 1 - Standards of fair-trading, process and transparency in the relationship between Insurers and Repairers; and Clauses 4.2(b) (iii) pay the agreed amount for all work completed that has been authorised or requested by the Insurer.

    • I therefore say that the insurer has breached both the Letter and the Spirit of the Code and I request that this matter be reviewed in a timely fashion and within the guidelines stipulated by the Code.
