A non-refundable deposit of $100 USD plus a $5 service fee is required for pre registration, with 75% of the balance due no later than June 28th, 2017 and the remainder by July 20th 2017. All deposits are non-refundable and necessary to secure your costume and participation in Carnival Xtascy Private Section ALLURE for Grenada Carnival 2017. Kindly note that once you have made your selection, there will be no switching or swapping. Costumes will be defaulted as of August 2nd, 2017, without full payment and will be placed back into inventory. NO EXCEPTIONS! Please note all payments are non refundable.
I understand that I am responsible for the payment of my selected options and costumes as laid out in this registration form and that I am fully understanding of the terms and conditions. I certify that there are no health-related reasons or problems which preclude my participation in this activity.I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity. I, the undersigned do herby consent and agree that Carnival Xtascy Private section ALLURE, its employees or agents, have the right to take photographs, videotape, or digital recordings of me and to use these in any and all media now or hereafter known. I release all rights to exhibit this work in print and electronic form publicly or privately and to market. I understand that there will be no financial or other remuneration for recording me.I represent that I am at least 18 years of age. If under the age of 18 years old you must bring a parent or guardian to sign a release form, before the costume is released.
