This Part 1 – Register of Interest is one of the official documents of submission to be completed by Applicants in order to be eligible to participate in the Football NSW Futsal Competitions for 2016/17.

To be eligible to participate in the 2016/17 Competition, an Applicant must comply with the Application Process set out in this Part 1.

Part 2 sets out the Participation Criteria that will assist Football NSW in determining whether an Applicant will be granted a license to participate in the 2016/17 Competition.

Part 3 is the contractual arrangement between Football NSW and an Applicant that sets out the terms on which a license will be granted to a successful Applicant to participate in the 2016/17 Competition.

Parts 1, 2 and 3 together form the Application Documents.

Part 1 to be submitted to Football NSW by 5:00pm 1 April 2016.

1.     I am a person who is authorised to submit this acknowledgement form on behalf of the Club;

2.     On behalf of the Club, I have read and understood Part 1 of the application in relation to the 2016/17 Competition released by Football NSW on 7 March 2016; and

3.     The Club agrees to be bound by the Application Process as set out in Part 1 - Terms and Conditions.
