Adopting a pet may be a more involved process than you expected, and it is important that you understand two things: 1) the process is the same for everyone and 2) Everything we do is for the best interest of the pets in our care and is aimed at giving them the best possible chance of ﬁnding a permanent home. Dog/puppy adoptions require a minimum 24 hour wait period and there are times that other pets are not ready to leave our care immediately because of medical issues (illness, in need of spay/neuter) or delays in getting information from landlords/veterinarians. Before you can adopt a pet, you must complete this Pet Adoption Application, designed to help people understand the long-term responsibility of owning a pet and to ensure that all adoptions are in the best interests of the pets. Please ensure you are certain you are willing to commit to the pet for the pet’s natural life. We want the relationship between you and your new companion to be a long and happy one. You may have questions or problems that come up after you bring your new companion home and we want you to know that we are always happy to help you solve them. It will take some time for your new companion to become adjusted to a new home. The adjustment period can take as little as two weeks (or less) to several months. It is very important that you give your adopted companion adequate time to adjust to your environment and to go through the training process. Consistency, repetition, and lots of praise are important concepts to remember in training your new companion. THE MINIMUM AGE TO ADOPT IS 19 YEARS OF AGE.

In some instances we allow some dogs/puppies to go home before they have been altered. DISCLAIMER: The CCSPCA makes a commitment to give full disclosure on all pets in our care. We evaluate pets as adoptable based on behaviour at the time said pet is under our care. If the pet was surrendered by an owner, all information that the previous owner has given us will be passed on to potential adopters. In many cases the pets under our care come in as unclaimed strays, so no previous record of behaviour is available. The CCSPCA deems pets that have been evaluated as chronically ill or those who have been evaluated as dangerous are not made available for adoption to the public. We do everything in our power to see that our pets are healthy at time of adoption. From time to time there are health problems in pets that have not been diagnosed and may be found at a later date. The CCSPCA accepts financial responsibility for pets only while they are under our care. We accept no financial responsibility for pets once they have been adopted other than what has been agreed on by both parties. Potential adopters shall have the opportunity to have pets they may wish to adopt checked by their own veterinarian at their own expense before adopting. I HAVE READ AND UNDERSTOOD

ADOPTION FEE IS NON-REFUNDABLE: Should you not be able to keep this pet for any reason, it is your responsibility to aid in finding said pet a new home. The CCSPCA will take back pets at any time, subject to available space. A surrender fee may apply. I have read and understood.

I have read this entire document and fully understand both the contract and my responsibility to this pet. I am able and prepared to offer this pet a loving home for the duration of his/her lifespan. I understand that the CCSPCA has the right to deny my request to adopt a pet, and I authorize investigation of all statements in this application. I have read and understood.
