APS - EPSS PMI FORM
Items listed in this form meet or at times exceed requirements of NFPA 110. WARNING: This is an automatic starting piece of equipment with rotating components and electrical voltages present. extreme care needs to be taken when inspecting. Observe all posted warnings and applicable OSHA requirements.
The following inspection has been created by APS to meet or exceed requirements for equip. as established by manufacturer or NFPA levels 1 and 2 Insp.
Refer to following Action code(s) when filling out or interpreting form:
A-Annual, AJ-Adjust CK-Check CH-Change CL-Clean, M-Minor Insp., 0-Opt, R-Record,
T-Test, V-Visual Insp. (Visual includes, rust, worn out, missing hardware, missing equipment, vandilism or gnomes)
CAUTION* potential failure by collapsed hoses, plain rubber fuel hoses need to be replaced no later than every 3 years and all braided every 5 years
BE AWARE: If fuel filter housing has a hand priming pump or any other O-Rings, REMOVE and REPLACE AS NEEDED
PAY ATTENTION: All belts should be replaced at intervals not to exceed 5 years. Use Industry Standards for deflection and wear
PAY ATTENTION: All hoses should be replaced at intervals not to exceed 5 years; rubber jacket water heater hoses every 2 years, fabric braided 5 years
CAUTION/BE AWARE: Batteries top surface area are to be clean and free of debris on every PMI. Terminals are to be cleaned and treated with corrosion inhibitor.
WARNING/PAY ATTENTION: Battery load test should not drop more than 70% of its base voltage in order to prevent contact pitting of solenoids or relays as well as a cuase of starter motor failure.12Vdc approximately no less than 8.2Vdc and for a 24Vdc system 17.4Vdc.Verify and note what the starter draw is with a clamp-on DC ammeter and add as a note for future reference
PAY ATTENTION: Per NFPA 110 batteries that are over 30 months or 2-1/2 years in service must be replaced. Verify if customer requires automatic replacement prior to proceeding with remarks or notes.
PAY ATTENTION: Optimal float charge rates for lead acids will be 2.25 volt per cell and in units equipped with an equalizer it would be 2.4 volt per cell Nicad will be 1.4 float and 1.6 equalize.
IM{PORTANT: The automatic transfer switch is an integral part of this EPSS and should be tested to ensure proper operation. Unless already noted to proceed you must ask facility or site contact for permission to test. Facility will experience a momentary power outage and may experience a sudden surge of power which is an inherent condition that can occur during transfer.

BE AWARE: Retransfer time delay is recommeded to be set at a minimum of 15 minutes IAW NFPA 110 can be less for non Healthcare or life safety loads but should not be less than 10 minutes
TO ALL APS TECHNICIANS: A FOLLOW-UP FORM WILL BE REQUIRED FOR ANY DISCREPANCY REQUIRING ADDITIONAL WORK OR PARTS.
