Excursion Permission Form Grade 4 - Grade 6

Only children in Grade 4 - Grade 6 are eligible to attend this excursion.
Our service is governed by strict regulations in regards to adult/child ratios, therefore we will be taking one supervising adult for every 8 children and 1 supervising adult for every 5 children for any water activities. These ratios maybe altered at the Coordinator's discretion on any given day.

We will be traveling by seat belted buses. If you wish for your child to participate please completed the form as soon as possible. These excursion are subject to adequate bookings and maybe cancelled through lack of child attendance. All excursions have a risk assesment available for viewing at the centre. Please feel free to ask for a copy from our educators.

Activities at Venue: The kids head to Sunshine Coast to enjoy a variety of rides and fun activities, roller coasters, dodgem cars and Ferris wheels.
